At this time, I am offering individualized medical nutrition therapy (MNT) for adults living with diabetes, prediabetes and individuals seeking treatment for binge eating.
It is important for patients/clients to feel comfortable with members of their care team. Please encourage your patients/clients to review the website content to familiarize themselves with my approach.

To place a referral, please complete the following form and fax it along with the below items to FAX #: (541) 777-6066
All referrals must include:
- Completed referral form
- Recent chart note
- Lab values
- Most recent pertinent lab values, although; lab trends are appreciated
- Please be sure to include an A1C with diabetes referrals
- Demographics and Insurance information
- Any other relevant information/documentation
After I receive your referral:
- Documents will be reviewed and additional documents may be requested as needed
- I will call your patient/client to schedule a session
